About the role

Join our Housing Relocation Support Program, supporting public housing residents in North-West Melbourne affected by redevelopment. As an OT, you'll help residents transition safely and confidently into new environments by identifying and addressing the environmental, social, and functional barriers they may face.

You'll work closely with people from culturally diverse backgrounds, those with living disabilities, chronic or mental health conditions, or older adults, to create personalised and meaningful care plans. Trauma informed and culturally safe practice is central to this position.

Your duties include:

  • Support the ongoing review and improvement of the Housing Relocation Support Program to be responsive to the changing needs of residents
  • Intake as specific assessments, home safety management and complex care case management
  • Participate in multidisciplinary coordinated care activities to provide wholistic care

About you

  • Tertiary qualification in Occupational Therapy with AHPRA registration
  • Experienced in delivering trauma informed and culturally safe care
  • Ability to work effectively across a multidisciplinary service and with marginalised clients
  • Strong belief and personal alignment in cohealth Values and Mission
